INTRODUCTION WHAT IS  IoT-BASED EMS FLEET MANAGEMENT SYSTEM?

I ntegrating  IoT features into  existing medical  devices shall transform the healthcare  service especially in emergency case ,  bringing  high  value for  the patients  with chronic conditions, and those requiring  real time monitoring from the ambulance until reach hospital.  The main idea for this project is to develop an ambulance fleet management that help to organize the Ambulance Management Unit and monitoring the ambulance location in real-time tracking by the GPS system. The important part of this research is building a smartphone application which can perform the data recording for both parties, the caller, and the ambulance management respectively, and to detect the real time  location of the ambulance.

Referring to the current ambulance management system practice in Malaysia, which is managed by the operator takes a lot of energy and times especially when the request volume is high at one time, and it requires more hands to handle the requests.  Basically, the operator’s roles are to receive the call from user or patient, gather their information and location details, and record all the informations manually before allocate the nearest ambulance unit to serve the emergency case . Hence, aim for this project is to replace all the current works done by the operators to one systematic system. It can automatically gather and recorded all the informations from the caller, and all the data related such as the ambulance driver information, estimation time ambulance arrive at site and also estimation time arrival at emergency center. All these information gathered and stored in cloud in real-time basis and can reach for both parties especially the medical assistant at the emergency center to prepare all the necessary preparation to receive the patient.

Aim/Motivation :  T o  enhance the emergency medical care process from the response operation till to the hospital admission,  using  IoT platform  to autonomously  provide the  real  time data to the  emergency healthcare  professionals team  and the emergency care centers  can receive the data accurately,  substantially reduce the death rate. Objectives of this project: To develop a real-time fleet management system for emergency ambulance  by using  the Android Studio IDE and flutter extension. To  evaluate the timely provision of accuracy in  real time monitoring                  system in ambulance to emergency center at hospital .

The project presents a concept for improving emergency medical care process and services delivery . It further pursues the objectives to connect the unconnected health things to enable patient-centric emergency medical care delivery using the Internet of Things paradigm. The study is focused on a case of road traffic injury . Road traffic injuries are, according to the WHO report on the topic, the leading causes of death worldwide . The Global status report on road safety 2018 , launched by WHO in December 2018, highlights that the number of annual road traffic deaths has reached 1.35 million[ 1 ]. “Road traffic injuries (RTI) are on increasing in developing countries. Healthcare facilities are poorly equipped to provide the needed services”..

Figure shows the block diagram of the system which consists of five stages of the process connection starting with the mobile application. Both mobile applications are the user and driver application respectively, which is in this research using Android system.

The important part of this research is building a smartphone application which can perform the data recording for both parties, the caller, and the ambulance management respectively, and to detect the real time location of the device which is in this case is the ambulance  unit  and  timely update the  EMR. The  EMR is accessible to any emergency doctor during an emergency  transportation . This proposed project should  enable to autonomously, automatically, and  wirelessly  exchange data with the information desk at the emergency care hospital and any computer networking wireless devices that can ad hoc be registered and connected to the ambulatory emergency care information system for providing or collecting data . The  GPS unit used in this research will be the one that is implemented originally inside the smartphone user and a GPS module integrated with a  NodeMCU  ESP8266 microcontroller installed in the ambulance unit. An API Key from the Google Maps is required in this interfacing process perform in the Android studio and flutter as the medium of application builder and will store in the firebase as the cloud. The implementation of IoT telematics in this project can help the fleet operators to automate the ambulance tour planning to the emergency sites since the ambulance response time has a significant effect on the survival rate intuitively.

The main  page and maps view on driver mobile apps .  This main page is displaying the Google maps  view which  is being used for the function of the real-time location tracking. The maps  will show  the live view of the ambulance location along the way that it moves toward  to the  user destination.

In this page, the driver  will receive  the pop-up notification whenever there is ambulance request from the  user mobile  apps. The ambulance request pop-up notification that appears in this page  will show  the accept button option along with the user location. When the driver hit  the accept  button on the pop-up massage, the ambulance real-time location will be sent to the user  apps.

State Emergency Center will receive call 999 then distributed to the nearest Emergency Care Center. HUSM covered area  Kubang   Kerian ,  Pasir   Tumbuh  which is 7-10km radius. Emergency Center disburse Emergency Ambulance with standby Medical Assistant. In ambulance, MA will connect with Emergency Medical Dispatch at Emergency Center through walkie-talkie to inform the vital sign readings (BP, SPO2, Blood Pressure) and condition of patient. MCC will take note all the patient data in the specific form.  Once ambulance arrived Emergency Center, the MA will pass the emergency patient with the patient form data to  Registra  (Emergency Doctor and Head Nurse) to re-assess the patient. Then,  Registra  will evaluate the patient based on Vital Sign readings, patient condition and Pain Scale  to three level case (Red, Yellow and Green zone).

Potential commercialization: This fleet management idea for emergency ambulance tracking system proposed in this project in order to replace the old-fashioned method on handling the ambulance service request. With the integration of that mobile application and GPS module that placed in the ambulance unit to the cloud database, it looks well-organized and more efficient in data storing management to serve a better emergency service in the future. The deployment of IoT for EMS fleet management also can help boost efficiency, visibility, and manageability, while helping lower costs to the management..

The most precious time in these kinds of emergencies is when the patient enters the ambulance to the point of reaching the hospital and to get hospital staffs to  get  ready for treating the  emergency case casualty. Benefit to mankind  :  IoT-based  EMS fleet management is conducive for real-time monitoring response. It provides an alternative and robust method in developing a remote control ambulance tracking.  Iot's  self-driven automobile connections in this EMS ambulance fleet management provide secure and dependable real-time monitoring functions.  Novelty  and  uniqueness :  IoT-based  EMS fleet management has uniqueness especially in terms of the implementation of IoT telematics in this project can help the fleet operators to automate the ambulance tour planning to the emergency sites since the ambulance response time has a significant effect on the survival rate intuitively.
