[Audio] Stop Work Policy. Stop Work order shall be invoked if any situation arises due to an unsafe action /behaviour /omission or non-action of any party involved in the operation. Any personnel has the right and duty to invoke the Stop Work policy if in his opinion or judgment, such activity is deemed to be a potential incident. No employee shall be faulted or disciplined for invoking the Stop Work order when, upon investigation, it (Stop Work order) was found to be unnecessary. The Stop Work order must be applied in good faith. Timing is critical. There should not be any delay in calling for a Stop Work order if a potential risk or threat is imminent or the need arises All activity that has ceased due to this policy shall not resumed until it has met all safety protocol inline with the satisfaction of all at work.

CONFINE SPACE ENTRY Must obtain valid entry permit Adequate lighting & ventilation Ensure buddy system and a gas meter is being carried by competent gas meter carrier in the confine space Ensure gas test result is valid before entering O 2 – 20.9 ( 19.5% ~ 23.5% ) CO – 0% H2S – 0%.

Safe Work Procedures. Hot Work Activity Must obtain valid permit Ensure all flammables are away and protected Ensure hot sparks are contained Ensure availability of fire watchman and suitable firefighting medium.

Usage of Forklift and Self propelled Platform Designated and Qualified operator only. Machine to be registered and approved for use in yard. Use “approved checklist” for daily checking. DO NOT exceed safe working load and safe working capacity. Note : Forklift (headlight and beacon light compulsory) Self-propelled platform (overhead guard and outrigger fully extended compulsory).

Traffic Safety. Traffic Safety Only Land Transport Authority (LTA) registered and yard authorized ‘restricted use’ vehicles are allowed in the yard moving at 20km/ hr No entry to the work area unless issued with security vehicle pass Reverse parking at all times Walk along designated walkway.

Do note of the following: 3 mins continuous siren To alert those around you and proceed to Muster point Remove confine space card from tag board before proceeding to muster point. Walk briskly to the muster point in an orderly manner Head count by Supervisor and report to the warden / safety personnel Wait for Further instruction from Safety Personnel.

Hazard & Incident reporting. ALL INCIDENT INCLUDING NEAR-MISS MUST BE REPORTED TO QHSSE DEPARTMENT IMMEDIATELY & NOT LATER THAN 24 HOURS. Supervisor to follow up with an incident report upon completion of required first aid..

Vaccination shall be at least 3 doses. Practice Personal Hygiene.
