Although all EH&S are important and must be followed, Cardinal rules are rules that, if violated, have the potential for causing a fatality or serious injury. P&W sites are expected to communicate, ensure effective application, and enforce the Cardinal Rules. Cardinal Rules should not be singled out above other equally critical safe work practices, all rules are important and shall be complied with equally. Apply for a permit to enter confined space (e.g. Manholes; Chemical Tanks; Sump Pits; Oil Interceptors; Hydraulic platform’s pit in Test Cells, Utility Pits) Ensure all confined space workers and confined space rescue workers are trained Ensure all documentations for permit application are in place (Training certificates, rescue plan, calibration certificate, rescue equipment, gas meter, PPE etc.) Ensure positive ventilation is available prior to permit application To inform contractor coordinator immediately in case of emergency All competent persons are on site to assess and sign off PTW (confined space supervisor, confined space assessor & authorized manager) Employees must follow written procedures for entry into confined spaces. All confined spaces must be identified, labeled, and entry restricted and made safe for entry.
