Lock out / tag out teammate responsibility. When any piece of equipment, PIV, conveyor, etc. is not functioning properly, IMMEDIATELY let your Team Lead or Supervisor know. The Team Lead or Supervisor will then submit a request through Emaintenance. Maintenance Team will lock out equipment as needed with a tag indicating the reason. NEVER USE EQUIPMENT THAT HAS BEEN LOCKED!.
