What can’t the service provide? Reablement OT cannot provide support with housing adaptation. Please contact BB long term occupational therapist on 0123 456 7890

Our Reablement OT and Reablement worker support:
Triage by Occupational therapist
Assess patient need/ Occupation at home
Focus on patient goal by doing things together
Tailored care plan focused on rehabilitation process within 6 weeks service
Organising practical equipment including assistive technology to carry out goals in the manner of their choice
Help/ educate patient re learn daily skills
Promotes independence
Work closely with multi-disciplinary team- Physio therapist, Social worker, Gp
Regular review by an Occupational therapist

What if your patient need care after the service? Our Occupational therapist will advise the options to choose from and what patient may be charged.
