Determine the appropriate isolation points. Fill out the Isolation Check-List (APPENDIX B) Make the initial lock-out by placing Green Process Locks/Tags at the identified isolation points. Conduct a joint field inspection with Permit Receiver. Conduct a Field Show Me by trying to activate the isolated equipment. Place the Process Lock Key inside Lock-Box along with Isolation Checklist. Place a GREEN Department Lock on the Lock-Box. Fill the Sign On Sheet . Remove all locks/tags once the job is complete and once all worker locks/tags are removed..
