[Audio] Welcome to the Operating Theatre Fire Evacuation Plan in-service. During this in-service, you will be briefed on the roles and responsibilities of the fire wardens and the various operating theatre nurses during a fire activation, evacuation routes and the location of the fire extinguishers and hosereels..

Scene 2 (23s)

[Audio] These are the fire wardens. If there are no fire wardens around during the shift, EOT in-charge will assume the role of the fire warden..

[Audio] Right now, we will go through the roles and responsibilities of the fire warden and operating theatre nurses during the first fire alarm activation..

Scene 4 (44s)

[Audio] During the first fire alarm activation, the fire warden will check the location of the fire in OT on the mimic panel, which is located near the lift exit opposite OT 4. If there is fire in the operating theatre, the mimic panel will light up at the location of the fire..

[Audio] After checking the mimic panel and confirm that there is fire within OT, the fire warden will call and inform the fire command center ( FCC) 6 5 9 9 5, meanwhile others will assist to put out the small fire. If the mimic panel does not show any fire in OT and the PA system did not mention anything about the fire, fire warden will call FCC to confirm if it is a false alarm, standby or if there is real fire. In the event of a real fire activation, fire warden will inform OT staff and OT SNM/ SNC/ NC about the fire location and standby for further instructions. The fire warden will don the fire warden vest and carry the fire evacuation bag..

[Audio] There is a total of 5 fire evacuation bag in OT. 4 bags can be found in equipment room 3 pigeon holes and 1 bag is kept in PACU 2..

[Audio] Fire warden will need to retrieve staff roster, assignment file and visitors/ vendor file for attendance checking after the evacuation. If evacuation is needed, staffs will be assigned to assist in the OT. OT schedules can be access from EPIC. Lastly, fire warden will call Jurong Community Hospital unit sister to unlock the door leading to C 3 from PACU 2..

[Audio] Responsibilities of the circulating nurse. During the first fire alarm activation, circulating nurses have to ascertain if the fire alarm is real and confirm the location of the fire. Then, they have to inform the respective surgical and anesthesia team. Circulating nurses have to start preparing the consumables required to perform wound closure, obtain the hardcopy of swab count sheet and start recording the counts and note down all event timings to document on the hard copy forms later..

[Audio] Responsibilities of a PACU nurse PACU nurses have to prepare oxygen cylinders for patients who require oxygen therapy, turn off the medical panel when patient is ready for evacuation, ensure the IV lines are secured properly on the patient, detach the monitoring device and continue monitoring patient's vital signs during transportation, and lastly note all the event timings for documentation later on..

[Audio] Responsibilities of an OT technician O T T have to ensure all unused equipment in the OR are moved aside to facilitate a smooth evacuation passageway. When patient is ready for evacuation, O T T will switch off all the electrical equipments in the OT..

Detach the monitoring device from the anesthesia machine and continue monitoring patient's vital signs during evacuation. Secure all the patient's IV lines, carry the resuscitation drugs and emergency kit and bring along the syringe pump with the patient. Follow fire warden's instructions to the assembly point..

[Audio] There will be two main routes for evacuation The first one will be the horizontal evacuation for OT staff with patients. The assembly point will be in Jurong Community Hospital, Tower C, ward C 3. Staffs evacuating patients from OT, will go through the OT corridor and exit through the fire exit door near OT 11. Staffs evacuating patients from PACU will exit through PACU 2 fire exit door to the sleep lab, then to tower C. The second one will be vertical evacuation for OT staff without patients. OT staff will evacuate to the assembly point at Tower A, level 1, outdoor porch via any nearest fire exit doors..

[Audio] If there is a fire in tower B, the assembly points for horizontal evacuation of patients from Tower B will be in Tower C. If there is a fire in tower c, the assembly point will be in tower B.

[Audio] There are a total of 38 fire extinguishers available in the operating theatre and they come in different types for different classes of fire. There are 15 halotron for all classes of fire, 6 dry powder for class A and B fire, and lastly 17 carbon dioxide for class C fire..

[Audio] These are two very important acronyms that everyone have to remember. When there is a fire, we must follow these steps. The first acronym is race. R. - Rescue anyone in immediate danger of the fire. A. - Activate the nearest fire alarm. C. - Contain the fire by closing all doors in the fire areas. E. - Extinguish the fire only if it is safe to do so. If unsafe, evacuate the area. next important acronym is pass. When the fire is small and safe to extinguish, this is how you operate the fire extinguisher. P. - Pull the pin. A. - Aim at the base of the fire. S. - Squeeze the lever. S. - Sweep from side to side until the fire is extinguished. Remember, your safety is the utmost priority. Assess the situation properly and act accordingly..

[Audio] There are a total of 6 hose reels available in the operating theatre. These are the steps to operate the fire hose reel. First, turn on the hose reel valve. Next, pull out the hose and go to the location of the fire. Lastly turn on the water at the nozzle and direct the stream at the base of the fire. It is important to take note that hose reel cannot be used for electrical fire..
