Nursing Theorists: Dorothea Orem & Madeleine Leininger.
[Audio] Dorothea Orem's Self Care Deficit Nursing Theory (SCDNT) of nursing presupposes that the patients require the nursing intervention as they cannot meet care needs themselves. The theory comprises of three interconnected components, self care, self care deficit, and nursing systems. The three nursing systems Orem speaks of are wholly compensatory, then partially compensatory in which nurses and patients work together to assist in certain specific tasks, and finally the supportive-educative in which nurses help patients learn how to take care of themselves. Two types of self care bases are outlined a) basic air and water [they must have], and b) additional because of growth or illness. Orem's theoretical framework is widely used in treatment of chronically ill patients in conjunction with rehabilitation and old age services because individual patient autonomy is at the focus. She stressed on the need for nursing to be culturally competent and provide effective and meaningful care to patients from diverse backgrounds. In three ways, Leininger articulated culturally congruent nursing care: Cultural preservation (maintaining good health practices), cultural accommodation (adapt'ng care to cultural needs), and cultural repatterning (changing harmful cultural practices for improved health end results). The Sunrise Model offered by her assesses cultural factors in nursing care. Theory in Leininger's theory is widely used in global health, community nursing, and multicultural patient care. Since it considers cultural beliefs and practices in nursing interventions, it is particularly useful for addressing health disparities and increasing patient satisfaction.
