[Audio] 1.Access to Care- Patients must have access to the services they need, whether it's emergency, routine, or specialized care. 2. Availability of services- We have emergency services, outpatient care, inpatient care, and specialty services. This may also include providing services after hours, or having agreements in place for services not available at the facility. 3.Timeliness of Care:-Systems should be in place to manage patient flow, triage patients according to urgency, and minimize delays in treatment or access to care. 4. Patient Rights and Information: Patients and family are provided with information at admission. Patient to be oriented to room and services. Patient’s physical, cultural and language barriers are.

Continuity of Care-The healthcare organization is responsible for ensuring that care is continuous, which includes coordinating care between providers, particularly when patients transition between different levels of care, such as from hospital to home care or from one healthcare provider to another. 6. Emergency Services- Facilities must provide adequate emergency services, and patients should be able to access these services 24/7. There must be effective communication and coordination among healthcare professionals in emergency situations to ensure that timely, quality care is provided. 7. Referrals and Transfers- Staff should know referring patients to specialists(Woundcare,nurse,dietitian,PT,OTete) or transferring them to other healthcare facilities when needed. These processes should ensure that the patient receives timely and appropriate care, and that all necessary information is transferred with the patient to avoid gaps in care. Staff should know the process of transfer (the arrangements and transfer consent).

Accessibility for Vulnerable Populations-The facility should ensure that its services are accessible to all patient populations, including vulnerable groups (e.g., elderly, disabled, or economically disadvantaged individuals). This could involve adapting the physical environment and providing services that meet their specific needs. 9.Discharge planning - Staff should know general discharge Policy. Discharge planning starts on admission and should be ongoing. Patient should receive discharge inspections about(Medication, diet, nutrition, pain management and ADL).
