[Audio] Open the Track Entry tab. Fill one row for each observation. Use the dropdown menus for Type, Severity, Area of Focus, Status, and Department. For Description and Action Required, type freely. After entering data, press Ctrl+Alt+F9 to refresh all calculations..

[Audio] When you click the Type cell, a dropdown appears. Choose the type that matches your record. Observations and Near Miss use Unsafe Act or Unsafe Condition. Audit findings use NC. Drills use Drill Finding or Observation. Committee decisions use Decision. Accident investigation actions use Corrective Action..

[Audio] Every record needs a Severity — High, Medium, or Low — and a Status. Open means not started. In Progress means someone is working on it. Closed means the action is done and verified. Important: if the Completion Date passes and the Status is still not Closed, the entire row turns red automatically. That is your overdue alert..

[Audio] Go to the Chart Data Source tab. You only need to change two cells. Cell B3 is the Analysis Year — type the year you want and every dashboard updates instantly. Cell E3 is the Type Filter — type All to see everything, or type a specific type like Near Miss to filter all charts. That is it. These two cells control the entire tool..

[Audio] The Summary Dashboard gives you a complete picture at a glance. Six key numbers: Total Records, Open actions waiting, In Progress actions being worked on, Overdue actions past their deadline, High Severity open items, and Closed completed actions. Below that you will see the top areas of focus that need attention, and trend spikes by quarter..

[Audio] The Monthly Dashboard compares each area of focus this month against last month. The Delta column tells you everything. Red Up arrow means more observations this month — something is getting worse and needs investigation. Green Down arrow means fewer observations — improvement. Yellow Flat means no change — check if you are under-reporting..

[Audio] The Trend-Based RCA tab is where you investigate why observations increased. Columns A to F are calculated automatically — never edit them. Column E shows the trend direction. When you see an Up arrow in red, that is your trigger to act. Now fill columns G through L. G is the Root Cause — why did it happen. H is the Correction — what you did immediately. I is the Corrective Action — the systemic fix to prevent it happening again. J is who is responsible. K is the target date. L is the current status..
