Patient must meet the definition of homebound

 Needs help from another person or medical equipment to leave home or leaving could worsen their condition.

 Leaving home is difficult and generally limited to short medical trips or necessary outings.

 Under the Care of a Physician

 Patient must have a physician who establishes and signs the Home Health Plan of Care (POC). Physician certifies homebound status and authorizes services.

 Services are Medically Necessary	

 Care must be skilled and required for treatment of illness or injury.

Files on shelves. Record Retention. CMS has a retention policy of five years unless the state law states a longer time. Oregon's OAR 333-027-0150 stipulates 7 years of retention. Since the State of Oregon requires a longer record retention time than CMS, we will follow state guidelines..

Certification is the physician’s order that a patient qualifies for home health services. This is required for payment from Medicare.

 Initial certification covers 60 days of home health services.

 Recertification required every 60 days if home health services continue.

 Physician must review and update the Plan of Care and confirm homebound status and medical necessity.

 Face-to-Face Encounter Requirement

 Within 90 days prior to certification OR within 30 days after certification

 Telehealth may be used as a supplement in some geographic or clinical situations.

An RN must complete an initial assessment within 48 hours of referral, the patient’s return home, or the physician’s start-of-care order.

A comprehensive assessment must then be done within five days of starting care. The assessment must be updated:  every 60 days, between days 55–60  within 48 hours after a hospital stay of 24 hours or more at discharge.  Documentation at discharge must clearly support the decision to end services
