[Audio] DRILL TYPE : Emergency Evacuation Drill - Medical LOCATION : Zone 1, Greenfield Area, Cristal MGT Project DATE : 12 July 2018 TIME : 05:00 PM PURPOSE The purpose of this drill is to assess the preparedness of the emergency response team and to practice the roles and responsibilities of HSE personnel and key personnel in the event of a medical emergency situation. OBJECTIVE To familiarize ERT, HSE and key personnel on the actions to be taken in the event of a medical emergency situation. SCENARIO One (1) worker from Piping Group slipped and fell down from a ladder of 5 meters high in Zone 1, Greenfield Area while he was descending and at the same time carrying a grinding machine on his left hand. The person was consciously lying on the ground when he was seen and noticed by his foreman. A minute after, the HSE Officer at site began to contact the HSE Lead to inform the incident. 05:01 P.M. - HSE Officer called the HSE Lead to inform the incident. 05:02 P.M. - HSE Lead alerted the ERT personnel and the ambulance. 05.03 P.M. - ERT personnel aboard ambulance proceeded to the incident area. 05.04 P.M. - ERT personnel and the ambulance arrived at the incident area. 05:05 P.M. – ERT personnel conducted medical rescue and started transporting the patient to the nearest hospital. 05:06 P.M. – Conducted muster/personnel accounting. COMMENTS ACTION REQUIRED 1 Good communication and reporting procedures between the site HSE officer /foreman and the HSE Department were observed. Maintain 2 No site nurse and lack of qualified first aider sto assist during the drill and actual incident. To be provided 3 Lack of medical equipment like AED and first aid kits. To be provided 4 Need to provide a flagman at the incident location to guide ambulance. Assigned the trained flagmen to do the Traffic control and to assist in directing the ambulance to the incident area. 5 Workers need to be educated before they go to the assembly point they need to look for the wind sock to know the wind direction. To be discussed in a daily toolbox topic. 6 The Drill was well planned and coordinated. Maintain 7 The time and coordination was quick. Maintain.
