What is the DDD? The Department of Developmental Disabilities (DDD) is a state agency that provides support and services to children and adults with developmental disabilities to help them live as independently as possible. Core Mission DDD's mission is to empower individuals with developmental disabilities by offering services that promote self-sufficiency, inclusion, and the ability to thrive in the community. Eligibility Individuals must meet specific criteria, such as having a developmental disability diagnosed before the age of 22, to be eligible for DDD services. The assessment process helps determine the appropriate level of support and services. Collaboration  DDD works closely with individuals, families, and community partners to ensure that individuals with developmental disabilities have access to the resources and support they need to thrive.

WHO QUALIFIES FOR DDD SERVICES?. mannaø. Diagnosed before age 18.

The child must have a disability that was diagnosed before the age of 18 and is likely to continue indefinitely.

Cerebral Palsy  Epilepsy  Cognitive/Intellectual Disability  Autism Spectrum Disorder other conditions that present similar functional limitations.

The disability must result in substantial functional limitations in several areas of daily living.

FUNCTIONAL LIMITATIONS CRITERIA. Self-care Ability to dress, bathe, or feed oneself Receptive and Expressive Language Ability to communicate effectively Learning The ability to acquire new information or skills Mobility Ability to move or navigate the environment independently.

Self-direction Ability to make decisions about personal care and  well-being Capacity for Independent Living The ability to live without significant assistance Economic Self-Sufficiency Ability to manage financial affairs independently (more applicable to transitioning youth/adults)

WHAT SERVICES DOES DDD PROVIDE?. Respite Care Temporary relief for caregivers of individuals with developmental disabilities, providing a break from the demands of caring for their loved one. Habilitation Services Skill-building programs that help individuals with developmental disabilities learn and improve their daily living skills, such as personal care, social interaction, and independent living. Therapy Services Physical, occupational, and speech therapy services to help individuals with developmental disabilities improve their physical, cognitive, and communication abilities. Attendant Care Assistance with personal care needs, such as bathing, dressing, and grooming, for individuals with developmental disabilities who require support with daily activities..

Nursing Support Medical care and nursing services for individuals with developmental disabilities who have complex medical needs, ensuring their health and well-being are properly addressed. Day Programs and Employment Services Opportunities for education, vocational training, and employment services for individuals with developmental disabilities as they transition to adulthood, promoting their independence and community integration.

APPLICATION PROCESS. Application Process The application process involves submitting documentation, undergoing a comprehensive evaluation, and meeting with a DDD case manager to determine eligibility and develop an individualized service plan. Appeal and Denial process Families can request that DDD re-evaluate the decision with additional documentation or evidence. If reconsideration doesn't change the outcome, families can request a fair hearing where an independent reviewer will hear the case..
