[Audio] Unit-1, COH Observations (Night) a worker was climbing in an unsafe manner near a visible deep opening. Proper access was not provided, and the worker was not wearing a full body harness while working at height. M/s Biswash Engineering Location-U#1,TG-4th floor lubricant drums were kept at multiple locations in unsafe manner which reflects poor housekeeping practices. U#1,TG-0mtr near -oil cooler 1B1.

[Audio] Unit-1, COH Observations The winch machine wire rope was in contact and rubbing against a pipe, creating friction during operation. [Audio] The staircase walkway was slippery due to water spillage. This condition creates a risk of slip, trip, and fall for workers using the staircase. [Audio] Found workers engaged inside the boiler at Pass-1 area, but no hole watcher was present at the site. BM Engg. #1 boiler 32 MTR Hot work is being performed without a fire watcher at the work location. BM Engg,SEAL AIR FAN.

[Audio] The wire rope of the winch machine is damaged at multiple locations. Despite this condition, lifting activities were being carried out using the same rope. U#1 ESP 1PASS A,NEW ENGG Material bag is placed on the scaffolding platform without proper securing, which may fall from height and create a falling object hazard. NEW ENGINEERING WORKS,ESP#01 inlet.

[Audio] Multiple lapses in the erected scaffolding A standard was not erected/ touched the ground. Ledger of the scaffolding to be projected minimum 6 inches outward.
