[Audio] I had noticed a few phenomenon but these two are the most significant that I am going to talk about..
[Audio] The first one is about the mismatch signature received from laboratory in specimen delivery book..
[Audio] Indication as shown in the image; Aspect of nurse: repeated handwriting; time consuming; Aspect of lab: Histo n micbi 2 locations; always sign wrong.
[Audio] Plan: easy to use, cost efficiency, time consuming, safetyness;; possibility of setup may involve IT dept, E&M, portering, lab;; evaluation in accordance to the index; on the feed.

Action plan to engage frontline staff in A&OT. Target: all nursing staff Goal: 100% specimens are sent correctly in the 3 weeks of trial of new system; apart from the frozen section specimen/ urgent specimen without GRS+ label..
