PROJECT INTRODUCTION & SCOPE :- The Dahej Effluent Disposal Pipeline (EDPL) Project is an EPC/LSTK contract awarded by Reliance Industries Limited (RIL) to MEKA GROUP OF COMPANIES for installation of a new offshore effluent discharge pipeline from the Landfall Point (LFP) to the Diffuser outfall in the Gulf of Khambhat. The project involves design, engineering, procurement, fabrication, installation, testing, and commissioning of a 3.6 km, 30” HDPE offshore pipeline with diffuser structure. The system discharges treated effluent water from the Dahej Manufacturing Division (DMD) into the Gulf of Khambhat. [Audio] Mandatory Site Safety Rules Use only inspected and approved tools; report defects immediately Use waste bin. Keep waste segregated. Report all near misses, unsafe acts, and incidents immediately Use mobile phones only in designated areas Stay clear of suspended loads and moving equipment Entering site under the influence of Alcohol is serious offence If you are unsure and feel unsafe don’t perform the work All new joiners, visitors and contractors must attend HSE induction Wear required PPE at all times (Helmet, Safety Shoes, Vest, Gloves, Goggles) No work without valid Permit to Work (PTW) and authorization Watch out for fresher's and visitors No operating equipment or machinery without valid license / certification and competency Stay away from Jetty / Coastal area unless permitted Smoking is strictly prohibited inside worksite.

[Audio] Mandatory Marine Safety Rules Ensure navigation lights, signals, and alarms are operational before marine movements. Keep decks clean and dry to prevent slips and falls. Verify that all lifesaving appliances (life rafts, lifebuoys, fire extinguishers) are in good condition and easily accessible Report oil spills, leakage, or pollution immediately — prevent discharge into sea. Stay clear of suspended loads and moving equipment. Follow vessel master’s or barge supervisor’s instructions at all times. Secure loose materials and equipment before vessel movement to prevent falling objects. Check weather and tide conditions before starting any marine activity. Always wear life jacket and safety helmet Access to vessel or barge only through approved gangway with handrails and safety nets. No unauthorized boarding of any vessel, barge, or tugboat. Maintain safe distance from mooring lines Use fall protection when working near open edges or over water. Take part actively in scheduled emergency response drills.

Hazard : Anything which has the potential to cause harm Risk : The likelihood of harm or loss occurring due to exposure to a Hazard Near Miss : An event that could have caused harm but did not. Accident/Incident : An unplanned event that causes or could cause injury, illness, or damage. RISK Worker employed inside unstable trench NEARMISS Worker narrowly escapes the trench collapse caused by excavator movement ACCIDENT Trench collapses and worker is crushed under soil HAZARD Excavator operating near unstable trench.

Using equipment without proper training Not wearing personal protective equipment (PPE) Operating machinery at unsafe speeds Blocking emergency exits Using mobile phones while working Improper lifting techniques Bypassing safety controls on machinery Working at heights without fall protection Horseplay in the workplace Ignoring safety procedures for hazardous materials An unsafe condition refers to any hazardous physical or environmental factor in the workplace that increases the risk of an accident. Wet and slippery floors without warning signs Poor lighting in work areas Exposed electrical wiring Defective or unguarded machinery Cluttered or blocked walkways Lack of proper ventilation in confined spaces Unstable scaffolding or ladders Improperly stored hazardous materials Broken or missing Safety Equipment High noise levels without hearing protection According to OSHA Unsafe acts cause around 88–90% of workplace accidents, while unsafe conditions cause about 10–12% Unavoidable Causes (Acts of God, etc.)~2% ..

[Audio] MEKA EDPL Project -Permit to Work System As per MEKA at EDPL project site Each day before starting work, the Permit Requester shall collect and sign for the internal PTW with the Permit Issuer. The permit issuer must be the Site In charge .and the Permit requester will be Discipline Engineer or Supervisor Before starting work the Permit Requester shall Conduct a Tool box meeting and carry out a workplace inspection and complete the inspection form "Monitoring of PTW and Work Area", which shall be issued daily with the PTW. Both documents shall be kept available for inspection at the point of work along with the Permit to Work package which shall contain all relevant documentation such as equipment checklist , Job safety Analysis , STA, HSE Plan, HIRA, lifting plan for Review. This Permit to Work (PTW) is valid for a period of 07 days unless otherwise stated, and must be signed and closed by the Permit Requester at the end of each working period. This PTW will expire at 18.00 hrs. Each day unless an extension is authorized by a PTW Issuer/HSE in charge IN THE EVENT OF A SITE EMERGENCY THIS PTW WILL BE AUTOMATICALLY SUSPENDED SEPARATE WORK PERMIT MUST BE USED FOR NON-ROUTING ACTIVITIES OR AS PER THE CLIENT REQUIRMENT.

[Audio] MEKA EDPL PROJECT ACTIVITIES – OFFSHORE AND ONSHORE SITE PREPARATION COLLER CASTING HDPE PIPELINE STRINGING BUTT FUSION WELDING EXCAVATION DREDGING HDPL PIPE LINE TOWING HDPE PIPE INSTALLATION COMMISIONING The HDPL pipe line works are classified as onshore work and offshore work, in onshore work construction ,excavation, trenching, stringing, welding coller casting will be carried out at the other side offshore works will carried out such as dredging, pipe laying, towing through tug boat, coffer dam construction , torqueing, sheet piling pre commissioning activities will be carried out in order to identify the hazards as our the Job safety analysis , risk assessment the signifient potential hazards are identified.

[Audio] SIGNIFICANT POTENTIAL HAZARDS IN THE WORKPLACE THE SIGNIFICANT POTENTIAL HAZARDS RISKS AND THEIR CONTROL MEASURES , As we seen in the previous slide there are number of activities and sub tasks are involved in our EDPL project There are various potential hazards and risks are also associated with this work activities such as Fire / Ignition of Nearby Combustibles Explosion / Gases Burn Injuries from Sparks, Spatter, Hot Metal Eye Damage from UV/IR Radiation (Welding Arc Flash) Electric Shock from Welding / Cutting Equipment Fumes / Toxic Gas Exposure (Welding fumes, Metal fumes) Hot Work Spatter Igniting Confined Noise from Welding / Cutting / Grinding Flying Particles (Grinding Cutting Discs Burst) Cylinder Explosion (Oxy-Acetylene / Gas Cutting) Poor Housekeeping / Trip Hazards , Chemical hazards Fire Watch & Isolation: Clear and remove combustible materials within minimum 15m radius. Use fire blankets/screens. Hot Work Permit: Obtain and enforce a valid permit before starting work.Gas Testing: Test area for LEL before and during work. Ventilation: Maintain forced ventilation. Isolation: Shut off and blank fuel sources; avoid hot work if flammable vapors detected. PPE: Fire-resistant coveralls, face shield/goggles, welding gloves, sleeves, apron, safety boots. Barriers: Use spark containment guards. Tool Safety: Proper electrode and torch handling. Eye Protection: Use approved welding helmet with correct shade lens. Barriers: Flash curtains/screens to protect others in the vicinity. Equipment Inspection: Check cables, clamps, insulation. Grounding: Ensure proper earthing/ground connection. Dry Conditions: Keep hands/gloves dry; avoid working on wet surfaces. Ventilation / Extraction: Use local exhaust or forced air ventilation. Respiratory Protection: Use respirator suitable for toxic metals (e.g., zinc, lead). Permit & Monitoring: Special permit for confined spaces. Continuous gas testing & standby attendant. Maintain fire watch for minimum 60 min after work. Hearing Protection: Use earplugs or earmuffs; restrict unnecessary access. Tool & Disc Inspection: Use right disc rating and guard covers. Face Shield & Goggles: Mandatory projectile protection. Ensure proper stance & grip. Cylinder Handling & Storage: Secure upright; maintain 3m separation between oxygen & fuel cylinders. Use flashback arrestors and leak check before use. Work Area Control: Manage cables/hoses, maintain clear walkways, properly store tools, keep surfaces dry and tidy..

[Audio] PRECAUTION AND CONTROL MEASURES In order to ensure safe and healthy work place each employees and workers including sub contractors of MEKA group of companies should actively involve to prevent incidents , injuries, property damages to MEKA Group hence we should practice safe procedures always never bypass or taking shortcut due to external work pressure every employee at MEKA should follow the safe system of work to execute the project successfully and safely .Every one should adhere safety signage, posters, warning boards , ensure Proper Lifting Techniques & Training Train all personnel in correct manual handling techniques (e.g., squat/stoop lifting, keeping the load close, avoiding twisting). Mandate a pre-task risk assessment for every manual lift. Clear Pathways & Good Housekeeping Maintain a clear and level path free of obstructions (tools, cords, debris). Ensure adequate lighting. Clean up spills immediately. Use a spotter for large, bulky loads. Clear Communication & Awareness Communicate intent clearly before moving ("Coming through"). Ensure doorways and aisles are wide enough for the load and person. Avoid blind corners. Job Rotation & Rest Breaks Implement job rotation to vary tasks and muscle groups used. Schedule regular rest breaks to allow muscle recovery. Use team lifts for heavy or prolonged carrying tasks.. FOLLOW SAFETY RULE FOLLOW SOP PRACTICE PPE’s ADHER SIGNBOARD.
