What is a DDP-2?   It includes a set of questions that provides a thorough description of an individual’s abilities and challenges as they relate to their service needs It is comprised of three sections of needs:  medical, behavioral and level of care The information determines a numerical score which identifies the level of need and support a person will be eligible to receive. The assessment is completed by the organization providing the service.  There should be a DDP-2 completed for each type of service received upon intake and every 2 years. Who does it affect? It affects ALL individuals receiving OPWDD related services.

Why is this important to understand? The DDP-2 collects information used to describe, plan and manage a person’s OPWDD services outlined in the life plan. The overall score directly affects the type and amount of services approved commonly referred to as the level of service. When will this happen? All DDP’s should be updated a minimum of every two years OR when an individual’s needs change. It is currently unknown if/when the DDP-2 will be replaced by the Coordinated Assessment System (CAS).

All services providing a service For example if a person is enrolled in community habilitation, respite and  Dayhab  with PCCS we should have 3 DDP2s one from each program The person to fill out the DDP2 should have the most intimate knowledge about the person receiving supports i.e  DSP, program coordinator, FIC, agency broker  etc This can be filled out with support from their documents on file, discussion with the person/family and other supports.

The Developmental Disabilities Profile Form (DDP-2 form) should be completed:  Within thirty days of when an individual moves to a new program/service Whenever a significant change occurs to an individual’s characteristics At least every two years to update a person’s capabilities.

What is a TABS ID Number? The “Tracking and Billing System Identification Number”  (TABS ID) is the number assigned to an individual when they apply to receive OPWDD services.  It is used as an “identifier” - similar to a Social Security Number. What is a DDP2 or ISPM Score? The Developmental Disability Profile (DDP2) is a summary of an individual’s abilities and needs which results in an overall numerical “score”.   The score is called an ISPM #  (Individual Service Planning Model Number). The score combines a variety of information about a person’s ability for self-care their overall health any behavioral health needs they may have These numbers are used for things like getting approval for high needs funding, service authorization and more

In Choices Go to the person's name Go to tabs Inquiry  Choose DDP2 Choose the corresponding DDSP Open PDF At the end of each section there is a score and at the end there is a full view of the scores

In the document section in  eVero Make sure to include the date it was completed and when it “ends” & the program name - so that you can report on who is coming due! You can also look this up in choices but we require all DDP2s to be uploaded in  eVero  for compliance reporting

Go to reports Go to document status report  Choose DDP2 ( you can also do level of care) Choose program Click run

Always fill out the DDP2 and any assessment with the framework in your mind that you must respond to the questions from a non-bias, non assuming standpoint – the facts are what we must go use in order to complete the DDP2. Answer the questions as if the person would be alone with no prompting for their abilities and thinking about the most support they would need on their most challenging day for their behaviors.
