TASK TO CREATE NEW SYSTEMATIC HANDOVER SYSTEM IN EXCEL FILE WHICH CONSIST OF INDIVIDUAL EQUIPMENT HISTORICAL EVENT/TRACKING. ACTIVITY GATHER/TRANSFER ALL THE INFORMATION IN HARDCOVER BOOK AND CREW HANDOVER FILE TO NEW EXCEL FILE. GTG-7510: failed to RUN due to WHRS byass fail. Stroking time exceed the timer. continue troubleshoot on WHRS damper bypass failed and T5 high. I. WHRS Damper bvpass Failed: caniblize pneumatic pilot valve from GTG-7520 due to original pilotvalve found sticky. Performed function stroke and rectified OPEN/CLOSED position as actual damper position. 2' J5_Higb: increase pressure (half turn) for air assist to torch. Pressure required abcwe PCD 30psi. Attempt START GTG-7510 successful and unit ready to load. out of calibration. Found travel sensor sticky and cause PCV overshoot to opening. Replaced travel sensor, Brrformed calibration and stroke test from PMCS. Now unit 0K. Assisst sedia teguh and Yokogawa on HMI upgrading. LCV-2031MB found leak at packing.Retighten and continue monitoring 0k. FT-6320 and FT-6330 reading O at site and DCS.Re-configure the data and online.ok LT-6401 found not tally with DCS.As-found bridle for LT-6401 was clog-Perform bridle cleaning assist by Deleum team. found 2kg of slug and the bridle.LT-6401 reading 0k. CTP A/B/C cannot run Auto from PMCS.As-found HS-6420 was on OFF position. Select on NORMAL mode and 0k. found unit not Found power supply was 6.8Vdc.Reset power supply and perform calibration-OK Troubleshoot on process pressure high. Remove actuator and found key way was not in proper place.re-setting the kewuay and perform stroking.BOL and 0k. XV-2122 indication fault.Found water ingress.replace new limit switch and 0k XV•2112 unit Fail to open-service control Panel for XV•2112 and 0k. Finding the valve was hard to close.•Already inform Instrument Engineer MR Yoga. FCV-2331(Unit was not use since commissioning) condition at PMCS 0% but at site IW%.Found travel sensor was KAPUT. Replace new but fail to calibrate.Notice the assembly of tubing was at wrong position. •to verify with Senior Jismi and akmar XV-2327 A indication faulty.Found solenoid was sticky. Service and stroke 0k. Perform PPM 12-14/10/202: XZT-8111D lop-Perform lamp module service and cleaning but fail. Replace new lamp module. 0k lop-perform lamp module service and cleaning.ok 030-QZT-8431 IOP-Perform Calibration and moitoring.ok QZT-8222 IOP alarm Voltage Low.Check power supply and found cable lux was corroded. replace and perform calibration.ok QZT-8312 and QZT-8110B IOP require scaffolding. QZT-8415C IOP.as found water Ingress.
