REPORT NE/2392/22/11/2025 IMPHAL CMP. FATAL INCIDENT IN IMPHAL CMP- NE CIRCLE.
aull 83 8u!8ueH MOI. Incident Summary: FRT Team Led by Splicer Anupam Das (PPRJ-Pending) assisted by Asst. Splicer Chandon Kumar Bardhan (PPRJ-Pending) was on Rengdil- Damchera span for restoration of cut on Aerial Fiber route. Patroller Mr Prajit Das was working with FRT Team as a helper. Team completed splicing/restoration Fiber Cut on Over Head Cable around 6:20 PM while it was almost dark outside. The patroller was inside the vehicle throughout the time while the other team members were executing the restoration activity. In parallel along the fiber cable there was an existing live 11kV line which was found sagging around 4m from ground. Post restoration of fault, patroller came out of vehicle and removed the ladder from the SOH pole. While removal of ladder, it got touched with 11kV line accidentally & Patroller got electric shock. Patroller was not wearing Class-1 electrical hand gloves but was wearing safety shoe. He got burn in his right palm. Splicer took him to the nearest hospital (4km) in the FRT vehicle with the driver. Doctor supported with CPR at 7.05 pm but no response received and was declared dead at 7.40pm. Lapses of Safety Process/Practices: Splicer & Asst. Splicer were deputed in Field without onboarding in SCRUM. They were not trained properly on Fiber safety and other training and deputed in Field In spite of the fact that 11 KVA LT Line is passing by, FRT team carried out work without Hazard Mitigation/sensitizing team mates including Patroller. Patroller did not take due Safety precautions and moved unfolded Aluminum ladder from the pole ignoring the nearby electric lines. Patroller not used mandatory PPE Class 1 electrical hand gloves. Team used Aluminum Ladder on SOH pole instead of Wooden ladder or FRP ladder. RCA: While removing Ladder from LT Pole, it got accidentally touched with 11KVA line . Since Patroller was not using Electrical Hand gloves, he got electric shock, which caused electrocution and resulted into fatality..
